Verification forms

Verification Forms have gone home or you will be receiving them today.  Please review and correct or add any changed information. It is required to have a new updated version on file each year.  There is also a FOIP and TECH form attached.  Please ensure you have filled each form out and signed them all.  these are due back SEPTEMBER 17TH.
